Location: Hanover
Matrix Medical Network seeks clinicians to perform in-home Comprehensive Health Assessments (CHA) for adults and geriatrics, reviewing health histories, medications, social factors, and risks during visits of up to one hour. You’ll perform clinical assessments, tests, and coordinate with care teams to close care gaps.
Role requires NP/PA licensure, board certification in an approved field, and ability to travel extensively across multiple states.
