Home Health & Hospice Agency

Visiting Nurse & Hospice of Litchfield County is seeking a highly organized, dependable, compassionate full-time Scheduler
, preferably with home health scheduling experience to join our Home Health & Hospice team. The Scheduler plays an essential role in coordinating patient visits and supporting our clinical staff to ensure patients receive timely, consistent care.

Responsibilities
  • Coordinate daily schedules for nurses, aides, therapists, and other clinical staff based on patient needs, staff availability, and geographic location.
  • Schedule admissions, routine visits, and follow-up care.
  • Communicate schedule changes with patients, families, clinicians, and leadership.
  • Monitor schedules throughout the day and adjust as patient needs and staffing change.
  • Assist with weekend, holiday, and on-call coverage.
  • Maintain accurate scheduling information in the electronic medical record (EMR).
  • Track missed, cancelled, and rescheduled visits.
  • Assist with entering referrals and coordinating onboarding of new patients.
  • Provide professional and compassionate customer service to patients, families, and staff.
Qualifications
  • Healthcare, home care, hospice, medical office, or scheduling experience preferred.
  • Excellent organizational, communication, and time-management skills.
  • Strong attention to detail and ability to manage multiple priorities.
  • Comfortable working in a fast-paced environment where schedules can change quickly.
  • Strong computer skills and ability to learn new systems.
  • Ability to work independently while being a collaborative team member.

Full-time, Monday through Friday position offering a competitive salary and comprehensive benefit package. Coverage for weekend & holidays needed.
