Job Responsibilities

Perform initial resident assessment within three days of admission and continue assessment on an ongoing basis.

Update resident care plans identifying resident needs and defining goals with observable and measurable outcomes.

Implement resident care according to medical/nursing care plans ensuring accurate delivery of care.

Administer medications as ordered by the physician in accordance with Waveny Care Network policy.

Follow up with healthcare providers, pharmacies, and residents' POAs to ensure timely medication supplies and refills.

Assist with treatment orders performed or ordered by the physician.

Maintain knowledge of residents' conditions and report changes to appropriate discipline.

Act as charge nurse for 8‑hour shifts, recognizing and meeting residents' medical/nursing needs.

Act as liaison between residents, family, and physician to provide continuity of care.

Perform direct resident care as assigned.

Ensure resident understanding of care plans, health habits, procedures, medications, and discharge planning.

Perform any duties within staff nurse's ability as assigned by the Director or Supervisor.

Identify, prevent, and respond to incidents of elder abuse, neglect, exploitation and report concerns per state regulations.

Maintain open communication with interdisciplinary healthcare team.

Document care accurately, objectively and promptly in the Electronic Medical Record.

Communicate personnel and resident condition information to Nurse Supervisor or designated nurse to ensure all resident needs met.

Ensure safe and infection‑free resident environment by recognizing hazards, maintaining equipment, and adhering to infection control procedures.

Use proper body mechanics to prevent injury.

Investigate and document incidents and follow up with providers and supervisors promptly.

Maintain ongoing communication with families, POAs, and healthcare providers to provide updates on residents’ status.

Monitor residents for behavioral changes, wandering, elopement risks, and safety concerns.

Act as a role model demonstrating positive qualities and attitudes.

Provide direction and support to CNAs and other licensed personnel in resident care.

Maintain cooperative team effort among staff members.

Assist in training and teaching of CNAs’ required skills.

Reorganize assignments as needed to adapt to unexpected changes without compromising care.

Function effectively in designated Nurse role per Nurse responsibilities and standards.

Participate in continuing education programs and mandatory in‑services as required.

Remain knowledgeable in methods, techniques and skills required in the field.

Keep abreast of changes in the field through reading journals and professional conferences.

Communicate changes in the job field to coworkers.

Create and maintain a warm, caring and calm environment throughout the facility.
