About This Role

As the Home Health Patient Care Manager, you will oversee and coordinate clinical services, ensuring high-quality care for home health patients in accordance with physician prescribed plans and applicable regulations.

• Pay: $98,648 - $147,972 annually
• Location: Wallingford, CT

• Provide clinical services within the scope of practice as defined by state laws.
• Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
• Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
• Ensure patient needs are continually assessed and care rendered is individualized.
• Review assessments and plans of care daily and consult clinicians with recommendations.
• Function in any home situation regardless of client demographics.

• Current unrestricted RN licensure in state of practice

• Home care experience preferred

• Current driver's license and vehicle insurance required
• Current CPR certification preferred or ability to complete within 90 days of hire
