As a member of the Whitney Center Health Center Team, the MDS Coordinator is responsible for the preparation of all Medicare A MDS assessments, oversight of non-Medicare assessments for timeliness and thoroughness, electronic submission of assessments, determination of RUGS levels, coordination of information to accounting and oversight of the census by performing the following duties. In this role you will interact with residents, family members, co-workers, professionals and other visitors.

The MDS Coordinator is responsible for coordinating and overseeing the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) process for Health Center residents. This position ensures accurate, timely assessments and documentation in compliance with federal and Connecticut regulations while supporting quality outcomes, individualized care planning, and appropriate reimbursement.

• Coordinate and complete MDS assessments in accordance with regulatory timelines and requirements.
• Manage the RAI/MDS assessment schedule and ensure timely completion and submission.
• Lead interdisciplinary care planning processes involving nursing, therapy, social services, dietary, activities, and other clinical team members.
• Review resident records and clinical documentation to ensure accuracy, completeness, and appropriate supporting documentation.
• Collaborate with clinical staff to ensure documentation accurately reflects resident conditions, services, and care needs.
• Support appropriate Medicare, Medicaid, and PDPM reimbursement through accurate assessment and clinical coding.
• Monitor regulatory requirements and maintain compliance with MDS 3.0, OBRA/PPS regulations, and the RAI User’s Manual.
• Identify documentation or clinical inconsistencies and work with the interdisciplinary team to resolve them.
• Maintain accurate records and submit required assessments to appropriate regulatory agencies.
• Participate in quality improvement initiatives related to resident outcomes, documentation, and reimbursement.

• Current, active, unencumbered Connecticut RN or LPN license

• RAC-CT (Resident Assessment Coordinator - Certified) certification
• Minimum2 years of MDS assessment/coding experience preferred.
• Strong knowledge of MDS 3.0, RAI processes, OBRA/PPS regulations, and PDPM

• Excellent clinical assessment, documentation, organizational, and communication skills.
• Ability to work collaboratively with an interdisciplinary team.
• Strong attention to detail and ability to manage multiple deadlines.
• Proficiency with electronic medical records and related clinical software.
