As a Licensed Practical Nurse, you will support the physician or mid-level provider with assisting in development, implementation and evaluation of patient care in the office. You will coordinate care with other services or outside health care providers and serve as a liaison between the physician, the mid-level provider and all other health care providers.

Provide clinical support to physicians and mid-level providers in the delivery of patient care in the outpatient setting. Coordinate patient care with internal and external health care teams to ensure continuity and quality of care. Communicate effectively with patients and health care professionals to support treatment plans and follow-up care.

• Practical nurse diploma or equivalent required
• Current LPN licensure or practice permit in the state of Pennsylvania required
