Position: Physical Therapist Assistant (PTA) Home Health Full-Time - North Houston, TX

Thera Care is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health opportunity providing in‑home physical therapy services to adult and geriatric patients throughout North Houston and surrounding areas.

Location:

North Houston Area.

Job Responsibilities
  • Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT).
  • Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT.
  • Treat patients with orthopedic, neurological, cardiac, and post‑surgical conditions.
  • Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities.
  • Apply manual therapy techniques and non‑complex modalities including heat, cold, electrical stimulation, and other physician‑ordered treatments.
  • Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies.
  • Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist.
  • Complete accurate and timely electronic documentation in compliance with Medicare home health regulations.
  • Follow infection control standards and home health safety protocols.
  • Participate in QAPI and agency performance improvement initiatives.

Qualifications
  • Associate degree from an accredited Physical Therapist Assistant (PTA) program.
  • Active Physical Therapist Assistant (PTA) license in the state of Texas.
  • Home health experience (1 year required).
  • Home Care Home Base  (HCHB) experience (preferred).
  • Current CPR certification.
  • Strong communication, clinical judgment, and time‑management skills.
  • Valid driver’s license, reliable transportation, and current auto insurance.
