Thera Care is seeking a licensed Physical Therapist Assistant (PTA) for a full-time home health opportunity providing in‑home physical therapy services to adult and geriatric patients throughout North Houston and surrounding areas.
Location:
North Houston Area.
- Provide skilled home health physical therapy interventions under the supervision of a licensed Physical Therapist (PT).
- Implement and carry out individualized Plans of Care (POC) developed and delegated by the supervising PT.
- Treat patients with orthopedic, neurological, cardiac, and post‑surgical conditions.
- Administer therapeutic exercises, gait training, balance training, transfer training, and functional mobility activities.
- Apply manual therapy techniques and non‑complex modalities including heat, cold, electrical stimulation, and other physician‑ordered treatments.
- Educate patients and caregivers on home exercise programs (HEP), fall prevention, and safe mobility strategies.
- Monitor patient response to treatment and promptly report changes in condition to the supervising Physical Therapist.
- Complete accurate and timely electronic documentation in compliance with Medicare home health regulations.
- Follow infection control standards and home health safety protocols.
- Participate in QAPI and agency performance improvement initiatives.

- Associate degree from an accredited Physical Therapist Assistant (PTA) program.
- Active Physical Therapist Assistant (PTA) license in the state of Texas.
- Home health experience (1 year required).
- Home Care Home Base (HCHB) experience (preferred).
- Current CPR certification.
- Strong communication, clinical judgment, and time‑management skills.
- Valid driver’s license, reliable transportation, and current auto insurance.
