To promote pharmaceutical care by managing the medication process, identifying and preventing potential drug-related problems, and resolving actual drug-related problems. Additionally, to perform the duties of a registered pharmacist as defined by. All responsibilities will be performed in a manner developmentally appropriate for the ages of patients served (0-adult).
- Ensures the appropriateness of all medication doses in reference to the patient's diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by lack of medication incidents.
- Accurately enters medication orders in computer as evidenced by lack of medication incidents.
- Ensures the medications the patient receives are correct as documented by lack of dispensing errors file.
- Ensures medication orders are processed and doses are delivered within established guidelines as evidenced by peer review or by the lack of medication incident/customer complaint.
- Follows aseptic technique and infection control in the preparation of sterile products. Insists technicians and other pharmacists practice aseptic technique and infection control. Completes annual re-certification and scores at least 90% on the aseptic technique/infection control skills and written exam as documented by a qualified observer.

• Participates in the education of patients, caregivers, and the community
• Supports patient safety systems
• Checks all available means of communication including e-mail, mailbox, and voicemail every shift
• Ensures the smooth running of the pharmacy and acts as a role model for technicians and other pharmacists as evidenced by lack of complaints, medication incident reports, and peer review
• Maintains investigational drug records according to departmental policy
• Contribute to the medication management process
• Conduct discharge medication review (difficult to obtain, prescription accuracy) and counseling, ensuring that patients/caregivers understand their medication therapies upon discharge
• Participates in study initiation, maintenance and closure

- Bachelor's Degree Pharmacy Or Ph.D. Pharm.
D. degree Required- RPH - Pharmacist State of Texas The pharmacist in this role shall be a licensed practitioner who holds a valid, unrestricted license to conduct clinical activities.
