• Doctorate degree in Physical Therapy OR Bachelor’s degree in Physical Therapy with passing the National Physical Therapy Exam (NPTE)
• Graduated from an accredited PT program
• Minimum one year's experience
• Active state license as a Registered Physical Therapist / PT or in the process of obtaining licensure
• Current CPR certification

• Assess referred patients requiring rehabilitation services
• Provide direct patient care to evaluate medical condition, functional capabilities, limitations, and potential for rehabilitation
• Establish and administer a treatment plan with specific goals based on patient capacity and tolerance under physician direction
• Set realistic, achievable goals and document patient status
• Educate patients and families on appropriate physical therapy methods
• Develop a plan of care and provide skilled interventions per physician orders
• Assess, plan, evaluate, and modify care to meet therapy goals
• Collaborate with interdisciplinary teams to set and evaluate patient goals
