Bilingual Child and Youth Wraparound Case Manager - Conroe
The Child and Youth Wraparound Case Manager provides intensive, individualized case management for children and families experiencing complex behavioral health challenges, with a strong focus on preventing out-of-home placement. Using the National Wraparound Initiative (NWI) model, the Case Manager assembles and leads a team of professionals and natural supports—including family members and friends—to develop and implement a single, unified plan of care.
The ideal candidate brings experience in child and youth mental health, thrives in a fast-paced environment, and is comfortable conducting in-home and community-based visits. The ability to remain steady and effective in high-stress situations is highly valued. This role carries a caseload of approximately 10 families.
After initial training, the typical schedule is Monday through Thursday, 9:00 a.m. to 8:00 p.m., with a one-hour lunch break and Fridays off. While the position is based out of our Conroe office in Montgomery County, it will serve families in Walker County as well as Spanish-speaking families in Liberty County as well. Mileage is reimbursed for work-related travel.
This is what you will do:
- Provide wraparound, intensive case management following the NWI model with high fidelity
- Provide assigned clinical eligibility assessments for YES Waiver Clients
- Billing and data entry responsibilities
- In and out of region travel required
- Writing strength-based family stories based on family input
- Complying with Tri-County and HHSC YES Waiver Policies and Procedures
You will need:
- Bachelor’s Degree in a Behavioral Science field or related discipline
- Must qualify as a Qualified Mental Health Professional (QMHP) under Texas Administrative Code (TAC) credentialing requirement (Rule §353.1415)**

- Bilingual in English and Spanish required
- 3 years MH experience and treatment strategies to effectively interact with consumers, families, and other agencies preferred
- Valid TDL and be insurable through Tri-County
