About the Role:
As a Caregiver at SYNERGY Home Care, you will:
- Provide one-on-one care to our clients, including assistance with activities of daily living (ADL's), medication reminders, safe transfers and mobility support
- Develop strong relationships with clients, families, and care teams to ensure seamless transitions and optimal outcomes
- Maintain accurate records of client care, updates, and progress
- Collaborate with healthcare professionals to ensure comprehensive care plans
- Demonstrate empathy, respect, and compassion in all interactions
- Participate in ongoing education and training to stay current with industry best practices

Requirements:
- 6+ months of experience in the as a caregiver, CNA, PCA, etc.
- High School Diploma or equivalent
- Current driver license
- Reliable and insured automobile
- Ability to pass background check and maintain confidentiality
