About the Role:

As a Caregiver at SYNERGY Home Care, you will:

• Provide one-on-one care to our clients, including assistance with activities of daily living (ADL's), medication reminders, safe transfers and mobility support

• Develop strong relationships with clients, families, and care teams to ensure seamless transitions and optimal outcomes

• Maintain accurate records of client care, updates, and progress

• Collaborate with healthcare professionals to ensure comprehensive care plans

• Demonstrate empathy, respect, and compassion in all interactions

• Participate in ongoing education and training to stay current with industry best practices

Requirements:

• 6+ months of experience in the as a caregiver, CNA, PCA, etc.

• High School Diploma or equivalent

• current driver license

• reliable and insured automobile

• Ability to pass background check and maintain confidentiality
