The Intake/Continuity of Care Case Manager will perform assessment and initial recovery planning services to individuals with chronic and persistent mental illness. This position's duties will include the following:
- Provides Continuity of Care, Case Management, and Rehabilitative Training for 20-25 adult individuals on Probation/Parole so they may achieve independent living and community integration
- This position has a small 1-10 intensive caseload with Huntsville parole.
- This is a community mental health position with the office as the primary work location. The typical work day starts and ends at the office and services are provided in the community and homes across the designated service area.
- Links adults to community resources and supports for Criminal Justice program requirements
- Responsible for providing all services using a trauma informed approach: creating a safe environment based on trust, transparency, collaboration, mutuality, and peer support while encouraging empowerment, and acceptance of voice and choice of individuals served, and demonstrating respect for cultural, historical, and gender issues.
- This position involves travel to clients’ homes and/or other community settings using personal vehicle.

This is what you'll need:
- Bachelor's Degree in the Human Services Field
- Must qualify as a Qualified Mental Health Professional (QMHP) under Texas Administrative Code (TAC) credentialing requirement (Rule §353.1415)**

- 3 years experience working with persons with mental impairment
- Clear criminal record validated by a TDCJ NCIC check prior to beginning work and annually
- Valid Driver's License and insurable through Tri-County
- Background in the criminal justice system
- Ability to pass computer literacy testing
- Experience with Microsoft Office software preferred
