The Coordinator of IDD Provider Services will oversee daily operation of Individualized Skills and Socialization site(s) (ISS) including contracted ISS sites, services within the community, in homes, Nursing facilities, and Host Home Services This position's duties will include the following:
- Serve as a liaison with members of treatment team to provide for the continuity and quality of services to individuals served
- Waiver Provider Coordinators to ensure assessments, implementation and monitoring of all services while ensuring compliance.
- Daily operations of 24-hour residential services ensuring staff available to provide the services
- Responsible for recruiting, selecting and training employees and contract staff to work within the different service areas
- Conducting Residential Monitoring Assessment / Inspections as assigned.
- Requires on call rotation as assigned due to 24 care assignment responsibilities
- As this is a program with 24-hour care services, hours must be flexible to assist outside of the normal 8-5 shift
- In region travel required
- High School Diploma or GED required
- Bachelor's Degree with course work in Social, Behavioral, Human Services, Social Science, Business or related field preferred
- 2 years of experience working with individuals with disabilities
- Supervisory experience preferred
- Experience and education may be substituted for one another, totaling at least six year's experience.
- Ability to pass physical agility test
- Valid driver's license and insurable through Tri-County
