Position: Nurse Practitioner / Physician Assistant
Affiliates of Family Medicine is looking for a full-time Mid-Level Provider-either a Nurse Practitioner or Physician Assistant-to join our healthcare team in our Conroe office. In this role, you will provide primary care services by evaluating, diagnosing, and treating patients while working under the supervision of a collaborating physician.

Position Summary:
The Nurse Practitioner/Physician Assistant is responsible for assessing, diagnosing, treating, and educating patients in a family practice setting. Working under physician supervision, the provider applies advanced clinical knowledge and skills to develop and implement treatment plans while monitoring and evaluating patient outcomes.

Essential Functions:

• Evaluate patients by reviewing medical histories and performing appropriate physical examinations, including comprehensive (360) exams
• Interpret patient data and collaborate with the supervising physician to determine appropriate treatment and management plans
• Prescribe and manage medications as appropriate
• Maintain accurate and timely documentation, including evaluations, treatment plans, progress notes, and other required reports
• Order and interpret diagnostic tests to identify abnormal conditions
• Coordinate referrals to specialists, healthcare facilities, and community resources as needed
• Ensure a safe, clean, and compliant work environment by adhering to all policies and regulations
• Perform additional duties as assigned

Requirements:

• Current Nurse Practitioner or Physician Assistant (PA-C) license
• Master's degree from an accredited Nurse Practitioner or Physician Assistant program
