What You’ll Do

As a Hospice RN, you will provide compassionate, patient‑centered care in the home or care facility setting while coordinating with an experienced hospice team.

Your responsibilities include:
  • Developing and managing individualized care plans
  • Providing symptom management and comfort‑focused care
  • Educating patients and families on disease progression and end‑of‑life care
  • Collaborating with physicians, social workers, chaplains, aides, and the interdisciplinary team
  • Supervising Hospice Aides and LPN/LVNs
  • Monitoring patient condition and communicating changes promptly
  • Completing accurate and timely documentation
  • Supporting families emotionally through every stage of care
  • Participating in on‑call rotation as assigned
Qualifications
  • Current RN License
  • CPR Certification
  • Valid Driver’s License & Auto Insurance
  • Hospice RN experience preferred
  • Home Care Home Base experience preferred
  • Strong clinical, communication, and critical thinking skills
