As a Hospice RN, you will provide compassionate, patient‑centered care in the home or care facility setting while coordinating with an experienced hospice team.
Your responsibilities include:- Developing and managing individualized care plans
- Providing symptom management and comfort‑focused care
- Educating patients and families on disease progression and end‑of‑life care
- Collaborating with physicians, social workers, chaplains, aides, and the interdisciplinary team
- Supervising Hospice Aides and LPN/LVNs
- Monitoring patient condition and communicating changes promptly
- Completing accurate and timely documentation
- Supporting families emotionally through every stage of care
- Participating in on‑call rotation as assigned
- Current RN License
- CPR Certification
- Valid Driver’s License & Auto Insurance
- Hospice RN experience preferred
- Home Care Home Base experience preferred
- Strong clinical, communication, and critical thinking skills
