Responsibilities
  • Delivers direct patient care including initial and on-going assessments of the patient
  • Develops and implements the Hospice Plan of Care in conjunction with the other members of the Interdisciplinary Team (IDT)
  • Provides education to patient/family on the hospice philosophy and care of the terminally ill patient and the patients’ individualized plan of care.
  • Works with Administrator/Director of Nursing to ensure appropriate staffing levels are maintained according to policy
  • Maintains primary control and management of each patient and acts as the primary liaison between physician, patient/family and the IDT
  • Supervises all patient care activity to assure compliance with current standards of accepted nursing and medical practice.
Qualifications
  • Minimum of two (2) years full-time experience as an RN, either in oncology, an acute care setting with direct patient care, a long-term care facility, hospice or home health care.
  • Current license to practice nursing in the state of Texas
  • Current CPR certification required
  • Home Care Home Base experience preferred
  • Current automobile insurance and valid driver’s license, maintain transportation to be used for work
  • Excellent patient assessment skills.
  • Good oral and written communication skills with an empathetic and people-oriented personality
  • Knowledge of managed care principles, and regulatory guidelines (i.e., Medicare, Medicaid, TJC, ACHC)
  • Proficient with technology and willing to learn new systems
