Develops and implements the Hospice Plan of Care in conjunction with the other members of the Interdisciplinary Team IDTProvides education to patient family on the hospice philosophy and care of the terminally ill patient and the patients individualized plan of care Works with Administrator Director of Nursing to ensure appropriate staffing levels are maintained according to policy

Maintains primary control and management of each patient and acts as the primary liaison between physician patient family and the IDTSupervises all patient care activity to assure compliance with current standards of accepted nursing and medical practice

Qualifications Minimum of two 2 years full time experience as an RN either in oncology an acute care setting with direct patient care a long term care facility hospice or home health care Current license to practice nursing in the state of Texas Current CPR certification required

Home Care Home Base experience preferred

Current automobile insurance and valid drivers license maintain transportation to be used for work Excellent patient assessment skills

Good oral and written communication skills with an empathetic and people oriented personality

Knowledge of managed care principles and regulatory guidelines ie Medicare Medicaid TJC ACHCProficient with technology and willing to learn new systems
