Pediatric Transitional Nurse – Registered Nurse (RN) / Licensed Vocational Nurse (LVN)

Conroe, TX | Bilingual Preferred (Not Required)

• LVN: $30–$35/hour
• RN: $35–$42/hour

• Provide skilled pediatric nursing 1:1 care in patients’ homes
• Perform patient assessments and ongoing evaluations; administer medications, treatments, and therapies
• Support hospital‑to‑home transitions for high‑acuity patients
• Educate and support parents, families, and caregivers
• Assist with discharge support and care coordination
• Train and support permanent field nurses during transitions
• Provide first‑shift and interim coverage as needed
• Collaborate with Clinical Care Managers and interdisciplinary teams
• Respond appropriately in emergency situations and changes in patient condition

• Full‑time position, with guaranteed hours
• Flexible scheduling based on patient needs and Nurse availability
• 5 working days per week, minimum 1 weekend day
• On call and Staffing coverage responsibilities as needed
• Local travel within approximately 30–40 miles

• Active Texas Registered Nurse or Licensed Vocational Nurse (LVN)/Licensed Practical Nurse (LPN) license
• Current CPR certification
• Ability to lift and assist pediatric patients as needed
• Experience with Trach care, Ventilator management, and TPN
• High‑acuity pediatric patients (preferred)
• Emergency response management

• Pediatric home health experience
• High‑acuity pediatric patients (preferred)
