Hospice RN Clinical Supervisor

Job DetailsSalary: $85,000 – $105,000 per year. Full‑time, stable leadership position with opportunities for career advancement.
Responsibilities- Deliver direct patient care, including initial and ongoing patient assessments
- Develop and implement the hospice plan of care with the interdisciplinary team (IDT)
- Educate patients and families on hospice philosophy, end‑of‑life care, and individualized care plans
- Supervise patient care activity to ensure compliance with accepted nursing and medical standards
- Serve as a liaison between physicians, patients, families, and the interdisciplinary team
- Partner with the Administrator / Director of Nursing on staffing and clinical operations
- Support clinical documentation, regulatory compliance, and quality patient outcomes
- Maintain compliance with Medicare, Medicaid, TJC, ACHC, and other hospice regulatory guidelines
- Provide leadership, guidance, and support to hospice nursing staff and field clinicians
- Active Texas RN license required
- Minimum 2 years of full‑time RN experience required
- Desired experience in hospice, home health, oncology, acute care, or long‑term care
- CPR certification required
- Home Care/Home Base experience preferred
- Valid driver’s license, automobile insurance, and reliable transportation required
- Strong patient assessment skills
- Strong communication, documentation, and clinical leadership skills
- Knowledge of Medicare, Medicaid, TJC, ACHC, managed care, and hospice regulatory guidelines preferred
- Comfortable with EMR systems and technology
