We are seeking a Hospice RN Clinical Supervisor to join a growing hospice team in Conroe, TX. This role is ideal for an experienced Registered Nurse with hospice, home health, oncology, acute care, or long‑term care experience who is ready to step into a clinical leadership role. The RN Clinical Supervisor will oversee patient care activity, support field staff, coordinate with the interdisciplinary team, and help ensure high‑quality, compliant hospice care.
The Hospice RN Clinical Supervisor will provide direct patient care as needed, complete patient assessments, support hospice plans of care, educate patients and families, and partner with leadership to maintain appropriate staffing and clinical standards.
Responsibilities- Deliver direct patient care, including initial and ongoing patient assessments
- Develop and implement the hospice plan of care with the interdisciplinary team / IDT
- Educate patients and families on hospice philosophy, end‑of‑life care, and individualized care plans
- Supervise patient care activity to ensure compliance with accepted nursing and medical standards
- Serve as a liaison between physicians, patients, families, and the interdisciplinary team
- Partner with the Administrator / Director of Nursing on staffing and clinical operations
- Support clinical documentation, regulatory compliance, and quality patient outcomes
- Maintain compliance with Medicare, Medicaid, TJC, ACHC, and hospice regulatory guidelines
- Provide leadership, guidance, and support to hospice nursing staff and field clinicians
- Active Texas RN license required
- Minimum 2 years of full‑time RN experience required
- Hospice, home health, oncology, acute care, or long‑term care experience preferred
- CPR certification required
- Home Care Home Base experience preferred
- Valid driver’s license, automobile insurance, and reliable transportation required
- Strong patient assessment skills
- Strong communication, documentation, and clinical leadership skills
- Knowledge of Medicare, Medicaid, TJC, ACHC, managed care, and hospice regulatory guidelines preferred
- Comfortable with EMR systems and technology
- Compassionate, empathetic, patient‑centered nursing approach
