Choice Hospice is seeking a Licensed Master Social Worker (LMSW) for our Conroe hospice branch in Texas. The LMSW will play a key role on the interdisciplinary care team, conducting psychosocial assessments and developing patient-centered care plans with families facing end-of-life challenges.
The LMSW will provide counseling, coordinate community resources, and advocate for patients while maintaining compliant documentation.
