The Advanced Practice Provider (APP) delivers high-quality, evidence-based care to patients in collaboration with physicians and other healthcare professionals. The APP conducts patient assessments, diagnoses conditions, develops treatment plans, performs procedures, and provides patient education. This role is critical in ensuring continuity of care and promoting patient wellness.
Duties/Responsibilities- Functions independently to perform age-appropriate history and physical for complex, acute, critical, and chronically ill patients.
- Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs.
- Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities.
- Implements interventions to support the patient to regain or maintain physiologic stability.
- Monitors the effectiveness of interventions.
- Facilitates the patient’s transition within and between health care settings, e.g. admitting, transferring, and discharging patients.
- Collaborates with multidisciplinary team members by making appropriate referrals.
- Facilitates staff, patient and family decision making by providing educational tools.
- Other duties as assigned.
- Strong computer skills including Word, Excel, and Power Point
- Eligible for hospital privileges
- Licensed Nurse Practitioner (NP) or Physician Assistant (PA)
- Graduate of an accredited Nurse Practitioner or Physician Assistant Program
- Current provider card in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
- PALS provider card as required by department and facility
- Capability to bend, stoop, and kneel to assess patients or perform procedures
- Ability to lift and carry items up to 25–50 pounds (e.g., medical equipment or assisting patients with mobility)
