Job Description

Licensed Practical Nurse (LPN) position at Creative Living, Inc. (CLI) serving residents in independent living services. The LPN works under RN supervision and is part of an interdisciplinary team, responsible for medication administration, health care of residents, instructional training, and documentation.

Job Summary

The LPN demonstrates competence in medication administration and ensuring adequate health care of residents, providing instructional training on self‑medication and sexual knowledge, and participating in interdisciplinary assessments.

  • Complete all necessary documentation to ensure compliance with state and federal regulations (daily data sheets, clinic notes, attendance logs, incident reports, behavior reports, etc.).
  • Maintain strict confidentiality of consumer information and facility operations.
  • Assist residents with ongoing health related skills, self‑medication, and sexual knowledge as indicated in IPP.
  • As an interdisciplinary team member, assess and report residents' health needs in IPP development and implementation.
  • Provide staff with in‑service instructions in health related areas as needed.
  • Document and complete forms or reports required by ICF‑MR regulations.
  • Maintain all medications and medication cabinet per ICF‑MR regulations.
  • Ensure first‑aid kits in the van and medication cabinet contain necessary items.
  • Follow through on medical orders and participate in all resident medical reviews.
  • Report medication errors with incident/accident reports submitted to RN and Administrator/QMRP.
  • Implement and maintain accurate medical records for residents.
  • Complete nursing assessment monthly.
  • Attend staff meetings as scheduled.
  • Coordinate weekend LPN work schedules.
  • Perform other duties as assigned within role scope.
Qualifications
  • Valid, active license for Licensed Practical Nurse.
  • Must operate a vehicle and hold a valid AR driver’s license in good standing; good driving record for last three years; ability to be insured as CLI driver.
  • Proof of current vehicle liability insurance.
  • Ability to lift at least 25 pounds.
  • Ability to observe behavior and determine needs from observations.
  • Ability to evaluate, review and maintain all records related to residents’ medical/health needs.
  • Ability to function within team process for assessment and program implementation.
  • Knowledge of training techniques and strategies for developmentally disabled.
  • Knowledge of sexual instruction.
  • Clear communication with staff and consumers via telephone and verbal means.
  • Follow necessary procedures in emergencies (fire, tornado, etc.).
  • Free from communicable diseases (TB skin test or chest X‑ray annually).
  • Experience working with people with developmental disabilities is helpful.
