The House Clinical Coordinator (HCC) is a registered nurse who is accountable for effective coordination of patient care needs. The HCC performs a variety of clinical and managerial duties within a hospital as assigned, coordinating care for patients and serving as a liaison between departments, patients, families, and healthcare professionals. The HCC assists with monitoring the delivery of patient care, resolves issues related to patient safety, clinical outcomes, or patient experience as notified by physicians or staff, and remains an active member of the multidisciplinary team.
The HCC collaborates in the development, execution, and evaluation of the multidisciplinary plan of care, considering ethics, safety, effectiveness, and cost. The HCC establishes goals and strategies to meet the continuing care needs of patients, families, and other care providers, ensures compliance with hospital policies, procedures, and quality standards, and provides care in collaboration with other nursing staff when needed.
The HCC participates in improving organizational performance activities, maintains nursing knowledge, and supports the professional development of self, nursing staff, and students. The HCC works with the team to assure that systems of care are in place for patients to move across the continuum of care, including development of patient care protocols, order sets, and efficient systems, responds to emergency medical situations, and evaluates the effectiveness of patient care.
- Registered Nurse (RN) license, typically required.
- Associate Degree of Nursing (ADN) or equivalent.
- Clinical experience: 2 years in a clinical setting, often required.
This job will be authorized 0.01 hours bi-weekly.
