Job Summary
We are seeking a dedicated, mission-driven Family Nurse Practitioner to provide comprehensive primary, preventative, and episodic healthcare to individuals and families in our medically underserved community. Working within an FQHC model, the FNP delivers culturally competent, high-quality care to patients across their lifespan, regardless of their socioeconomic status or insurance coverage.
Key Responsibilities- Underserved Care:
Provide full-spectrum primary care to diverse, vulnerable, and low-income populations across all age groups. - Chronic Disease Management:
Diagnose and manage high-prevalence chronic conditions including diabetes, hypertension, asthma, and obesity. - Integrated Care Integration:
Collaborate closely with on-site behavioral health specialists, dentists, and social workers for holistic patient care. - Preventative Health:
Perform well-child checks, adult physicals, immunizations, and cancer screenings aligned with federal quality metrics. - UDS Quality Metrics:
Document clinical data accurately to help the clinic meet federal HRSA Uniform Data System (UDS) quality measures. - Therapeutic Prescribing:
Formulate treatment plans and prescribe medications, leveraging 340B Drug Pricing Program benefits for patients. - Community Advocacy:
Participate in community health outreach, preventative health education, and population health initiatives.
- Education:
Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP) from an accredited institution. - Licensure:
Active, unencumbered Advanced Practice Registered Nurse (APRN) license in South Carolina. - Certification:
Board certified as an FNP by the AANP or ANCC. - Credentials:
Current DEA registration and active South Carolina controlled substance license. - FQHC Alignment:
Strong commitment to the mission of community health centers; experience with underserved populations preferred. - Cultural
Competency:
Exceptional communication skills with patients from diverse socioeconomic and cultural backgrounds; bilingual skills (e.g., Spanish) are highly valued.
- Schedule:
Full-time, Monday through Friday, with occasional extended evening hours or rotating Saturday clinics. - Loan Repayment:
Eligible for federal loan forgiveness programs through the National Health Service Corps (NHSC). - Malpractice Coverage:
