We are seeking an LPN Non-Institutional who is motivated to help individuals and families navigate life's challenges and achieve long-term wellness.

This position is located at the Department of Mental Health, Waccamaw Center for Mental Health, 164 Waccamaw Medical Park Drive, Conway, SC 29526.

Under direct supervision, the LPN plans, implements, and provides direct nursing care to all patients at the Conway office. The LPN collaborates with the treatment team to evaluate the effectiveness of care/interventions and discharge planning. Tasks include walking patients through the clinic, providing injections, taking vitals, dispensing medication in accordance with the SC Board of Pharmacy, and maintaining quality of care through collaboration with staff, nurses, and front‑office personnel.

The position is essential and may require reporting during emergencies or center closures.

Responsibilities
  • Provide direct patient care—administer injections, record weight, blood pressure, height, and pulse rate as directed by physicians and nurses.
  • Assist with prior authorization requests and seek physician assistance as needed.
  • Perform urine drug screens and other tasks requested by supervisor or Clinic Director.
  • Maintain patient care hours per productivity schedule.
  • Make observations, administer medication (oral and injections), educate patients and families through teaching and rehabilitative measures, and utilize other resources as appropriate.
  • Coordinate community support and work closely with Community Support Staff.
  • Complete all required paperwork for medication assistance, including logging medications, reviewing orders, and contacting clients for pick‑up. Submit clinical service notes within 24 hours per Local Collaborative Documentation Standards.
  • Maintain documentation of charts and records in accordance with State, Federal, Local, OMH, WCMH, Quality Assurance, Corporate Compliance, and CARF Standards.
  • Provide accurate technical and physical observations of patients and possess knowledge of practical nursing procedures. Experience in a mental health setting is required.
  • Understand services provided in comprehensive mental health programs for children, youth, and adults.
  • Serve as a backup for front‑desk check‑in and check‑out of patients when required.
  • Support medication stocking, triage patients when needed, and provide coverage for patients when other nurses are absent.
  • Follow up calls to physicians and nurses when completing lab forms and assist with referrals as needed.
  • Consult with nursing and medical team members when required.
Requirements
  • Graduation from an accredited school of nursing.
  • Must hold licensure as an LPN through the SC Board of Nursing (SC LLR).
Additional Physical/Functional Requirements
  • Ability to lift up to forty (40) pounds; ability to see, hear at normal volumes, stand, walk, reach, bend, stoop, pull, and climb at least two (2) flights of stairs.
  • Ability to tolerate noise level and circulate in patient areas.
  • Must be able to work with repetitive patient behaviors.
Preferred Qualifications
  • Bilingual abilities in English and Spanish (or another language) are a plus.
  • Candidates currently living in the catchment area are preferred.
Post‑Hire Expectations
  • Be able to provide a 10‑year driving record.
  • Be able to use a state vehicle for work‑related purposes.
  • Use clinical judgment while working independently and be self‑directed.
  • Communicate effectively.
  • Have the capacity to establish and maintain effective working relationships with patients and families.
  • Participate in ongoing in‑service training and other educational opportunities to maintain and acquire necessary skills.
