The Residential Care Provider monitors general resident activity to observe and report behavior/progress of individuals with severe developmental disabilities living in a group home or other community residential setting and provides individualized skills training according to residents needs.
Under direct supervision, assists with daily living activities in a residential facility. Provides training of basic personal and social skills, simple academics, or work skills to consumers/students in a state group home, workshop, or supported employment environment.
Duties & Responsibilities- Assist individuals with Developmental Disabilities in basic daily living skills.
- Carry out a person-centered Individual Service Plan (ISP).
- Ensure individuals are dressed neatly and appropriately and are always groomed according to VPH policy.
- Assist and encourage individuals to care for immediate personal needs such as toileting, washing their hands, and eating.
- Teach individuals daily skills such as household chores, laundry, cleaning, and washing dishes, as appropriate according to their individual plan.
- Transport individuals to work, day programs, doctor appointments, shopping, recreational activities.
- Assist with scheduling medical appointments.
- Assist in the implementation of individual and behavioral plans and provides feedback for plan goals.
- Assist individuals in the pursuit of recreational, social and community activities based on their preferences and individualized treatment plans.
- Assist and encourage individuals in the establishment of social networks.
- Promotes participation and involvement in community outings.
- Ensure everyone's rights are protected.
- Treat individuals with dignity and respect.
- Maintains the residence in a clean and organized condition.
- Other related duties as assigned.
- High school diploma/GED and six (6) months of relevant job-related training or experience.
- Experience working with IDD population.
- Certified Nursing Assistant certification (CNA), Direct Support Professional (DSP), Direct Care Professional (DCP) or Patient Care Technician (PCT) preferred.
- Must pass a pre-employment drug screen and subject to random drug screens.
- Must have a valid Driver's License and a clean Motor Vehicle Record in accordance with company policy.
- Must pass a criminal background check (including fingerprinting).
- Requires TB Evaluation at pre-employment and annually.
