MDS Coordinator
Classification:
Hourly Non-Exempt.
Reports to:
Administrator.
In keeping with our organization’s goals, the primary purpose of the MDS coordinator is to coordinate the Resident Assessment Instrument (RAI) process including completion of an accurate/timely Minimum Data Set (MDS), developing the interdisciplinary plan of care, and leading the daily Medicare Prospective Payment System (PPS)/Medicaid Case mix Meeting.
Responsibilities- Maintain current MDS status of each resident in accordance with applicable federal and state laws, rules, and regulations as well as facility policies and procedures.
- Schedule residents for appropriate OBRA, Medicare PPS, and/or Medicaid required assessments and communicate schedules to departments, families, and other stakeholders.
- Monitor clinical systems for changes in resident condition that may affect the MDS process and scheduling; coordinate MDS re‑assessment and re‑evaluation of the plan of care when needed.
- Set the assessment reference date (ARD) in conjunction with the interdisciplinary team to capture the resident’s needs, care, and services accurately.
- Complete sections of MDS and CAAs accurately in accordance with policies, procedures, and guidelines.
- Coordinate and attend daily Casemix Meetings, weekly Utilization Review Meetings, and monthly Triple Check Meetings.
- Maintain assessments on the active clinical record for a minimum of 15 months per Federal Regulations.
- Electronically transmit MDS data to the state in accordance with regulation and facility standards, policies, and procedures.
- Utilize the facility’s written policies and procedures governing day‑to‑day nursing service functions.
- Participate in facility surveys (inspections) conducted by authorized government agencies as required.
- Develop, evaluate, revise, and implement individual plans of care to achieve and maintain the highest practicable level of wellbeing for residents.
- Report any known or suspected unauthorized attempts to access the facility’s information system.
- Provide education to residents, families, and caregivers when needed or as directed by Administration or a physician.
- Deliver emergency support measures, including CPR, in accordance with physician orders, resident/family wishes, advanced directives, and facility policies and procedures.
- Submit complaint or grievance reports to the Administrator as required.
- Conduct oneself in a professional manner in compliance with facility policies.
- Provide care with respect for resident dignity and privacy.
- Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with facility policies and procedures governing accidents and incidents.
- Report suspected incidents of fraud, abuse, neglect, and/or misappropriation to the Administrator.
- Communicate and interact effectively with residents, visitors, families, staff, and supervisors.
- Attend and participate in departmental meetings and in‑services as directed.
- Report resident, staff, or other concerns to the appropriate department head.
- Report incidents, accidents, unsafe situations, and concerns immediately.
- Maintain knowledge of applicable federal, state, and local rules and regulations related to responsibilities.
- Ensure responsibilities are performed in accordance with all applicable rules and regulations as well as facility policies and procedures.
- Current, unencumbered, active license to practice as a Licensed Vocational Nurse (LVN) or Registered Nurse (RN) in the state.
- Registered Nurse license preferred.
- Preferred: 2 years’ experience in Long‑Term Care.
- Preferred: 6 months’ experience in rehabilitative and restorative nursing practices.
- Preferred: 2 years’ experience in completing the RAI process.
- Ability to make independent decisions when circumstances warrant such action.
- Demonstrated leadership and organizational skills with a positive and professional attitude.
- Strong knowledge of Point Click Care is preferred.
- Computer literacy with Microsoft Outlook, Microsoft Office, and Internet.
- Ability to work well under pressure, meet deadlines, and handle multiple tasks simultaneously.
