Kaiser Permanente seeks an LPN to deliver high-quality, tech-enabled patient care in a dynamic healthcare environment. In partnership with RNs and physicians, you'll perform hands-on clinical tasks, administer medications, and document care in advanced electronic health record systems. You'll support both in-person and virtual visits, help patients use digital tools, and contribute to workflow improvements. Responsibilities

• Provide direct patient care within scope of LPN practice, including vital signs, wound care, and medication administration under RN/physician supervision
• Assist with intake, triage, and documentation in Kaiser Permanente's tech-enabled clinical systems
• Use electronic health records and digital tools to document care, track outcomes, and coordinate with interdisciplinary teams
• Support virtual and in-person visits, helping patients navigate online portals and remote care technology
• Educate patients on care plans, medications, and use of digital health tools to manage their conditions
• Collaborate with physicians, RNs, and IT support teams to resolve workflow or system issues affecting care delivery
• Maintain strict adherence to clinical protocols, privacy standards, and regulatory requirements
• Participate in continuous improvement, giving feedback on digital workflows and clinical processes
