Kal Care Health Care is seeking an experienced, organized, and highly motivated LPN to join our growing clinical team as an LPN Care Coordinator & Clinical Education Coordinator.
This is a unique opportunity for an LPN who enjoys care coordination, caregiver education, clinical supervision, teaching, mentoring, and building relationships with clients and families
.
This is not a traditional bedside nursing position. The LPN Care Coordinator will serve as a key clinical resource within our home care program, working closely with our RN Case Manager, supervisory nursing team, caregivers, scheduling team, office staff, and agency leadership to support quality care across the organization.
What You'll DoAs our LPN Care Coordinator, you will help oversee the day-to-day clinical coordination and caregiver-development functions of our home care program.
Responsibilities- Coordinating and participating in new client Starts of Care
- Developing relationships with clients and responsible parties
- Providing ongoing client and caregiver clinical follow-up
- Conducting and coordinating supervisory nursing visits as needed
- Managing the supervisory visit calendar and helping ensure required visits are completed timely by supervisory nursing team.
- Coordinating with Kal Care's additional per-visit Supervisory LPNs
- Conducting caregiver orientation and clinical training
- Planning and delivering Kal Care caregiver in-services
- Conducting caregiver competency evaluations and performance evaluations
- Providing caregiver coaching, remediation, and retraining when needed
- Identifying caregiver educational needs and developing appropriate training
- Following up on concerns identified during supervisory visits
- Reviewing documentation and supporting quality-assurance activities
- Identifying changes in client condition and appropriately escalating clinical concerns
- Collaborating closely with Kal Care's RN Case Manager
- Developing Kal Care's monthly caregiver education and engagement flyer
- Coordinating our monthly caregiver engagement game/activity
- Helping create an organized, proactive, and engaging caregiver education program
We're looking for an LPN who is more than clinically capable.

- Enjoys working with CNAs, HHAs, PCAs, and other direct-care staff
- Communicates exceptionally well with clients and families
- Is organized and capable of managing multiple priorities
- Takes initiative without requiring constant direction
- Notices when something needs attention and follows it through to resolution
- Is comfortable providing constructive coaching and holding staff accountable
- Enjoys creating engaging educational experiences
- Has strong documentation and follow-up skills
- Can work independently while functioning as part of a clinical team
- Understands when a concern should be escalated to an RN
- Wants to play a meaningful role in improving the quality of home care
- Current and unrestricted Georgia LPN license
- Current CPR certification
- Strong clinical assessment, observation, and communication skills
- Reliable transportation and ability to travel to client homes as required
- Strong organizational and time-management skills
- Comfortable using electronic documentation and communication systems
Home care/home health, supervisory nursing, care coordination, geriatric care, caregiver education, CNA/HHA/PCA training, clinical instruction, competency evaluation, staff development, quality assurance, or nursing leadership.
Teaching or clinical education experience is highly preferred.Schedule & Compensation
Schedule: Approximately 32 hours per week / 4 days per week
Compensation: Competitive hourly compensation based on experience.
The LPN Care Coordinator will receive regular hourly compensation for all scheduled work. The position may also be eligible for an additional per-visit incentive when completing authorized supervisory nursing visits in accordance with Kal Care policy.
This position requires local travel to client homes. Mileage reimbursement is not currently provided.
