The ideal candidate will possess a bachelor’s degree in a human services field and have experience working with at‑risk youth. Service Coordinator

The Service Coordinator is a member of the professional childcare team and works with residents, families, and custodians to coordinate individual, group, and family therapy services. The position typically involves a forty‑hour workweek, with flexibility to meet the needs of residents and the program.

  • Coordinate and facilitate scheduled individual, family, and group therapy sessions
  • Be available for in‑home family meetings to enhance family engagement
  • Develop and complete Comprehensive Assessment and Individual Service Plans for each resident/family at intake and review within thirty days of admission
  • Gather information and documentation required by agency policy and regulatory standards prior to admission
  • Review Individual Service Plans as needed, not to exceed a ninety‑day period, and assess progress to redefine goals
  • Coordinate psychological and/or psychiatric evaluations and discuss resident needs with clinicians
  • Maintain case records in compliance with agency and regulatory guidelines
  • Facilitate participation of other team members in assessment, planning, and evaluation of resident and family/custodian needs
  • Maintain regular contact with referral sources, schools, adjunct service personnel, and other identified collateral resources
  • Facilitate the establishment and implementation of aftercare services for each resident and family or custodian
  • Carry out additional duties as assigned by the team and program administration

Qualifications:

  • Master’s degree in a human services related field
  • Minimum of 21 years of age and a valid Georgia driver’s license
  • Experience in child care, social work, or secondary education preferred
  • Dependable, emotionally stable, physically capable (sitting, standing, walking, driving, speaking, hearing, and physical restraining when necessary)
  • Strong decision‑making ability related to treatment issues, able to analyze and react to difficult situations under stress and pressure
  • Ability to work independently with minimal supervision
  • Skillful in building relationships with residents and parents/custodians from diverse backgrounds

Family Consultant (Conyers)

The Family Consultant works with foster families and children to ensure academic, therapeutic, and medical needs are met. Home visits are provided weekly or biweekly and the role functions as part of a 24‑hour/7‑day‑per‑week team.

  • Background in community‑based child welfare and experience with children exhibiting emotional and behavioral problems
  • Effective communication with clients, foster parents, and custodians from all backgrounds
  • Weekly or biweekly home visits and 24‑hour team support
