Overview

The Nurse Practitioner serves as a member of the Hospice Interdisciplinary Team and is responsible for the medical component of the Hospice Patient Care Program. Collaborates with the Medical Director and Attending Physician to provide certifications and recertification assessments, including physical examinations, medical history and physical, diagnosis and treatment, and prescribing medications as needed under the supervision of the Medical Director.

Responsibilities
  • Under the supervision of the Medical Director, provides direction and guidance to the hospice program, liaison with MD, Attending Physician, other NPs, staff and volunteers to assure the maintenance of quality care for patients and families.
  • Acts as a liaison between the hospice program, community physicians, Nurse Practitioners and others engaged in health care services to develop and expand the number of patients served by Hospice.
  • Provides direct medical care as needed to patients in their homes, nursing homes and in the hospital.
Qualifications
  • Graduate of a Master’s Degree Nursing Program with current ARNP license for the assigned state.
  • Experience in Nursing required, with Hospice or Home Care experience preferred.
  • Training, experience, and knowledge regarding the psychosocial and medical aspects of terminally ill patients; CHPN certification preferred.
  • Self-directed; excellent communication, organizational and interpersonal skills; ability to act as a liaison with the team and physicians.
  • Up-to-date knowledge of notices and formulary.
  • Current driver’s license for the state of residency.
