The Family Nurse Practitioner is a full-time, on-site role based in Cookeville, TN, providing primary care services to patients of all ages. Day-to-day responsibilities include conducting comprehensive health assessments, diagnosing and managing acute and chronic conditions, and developing individualized treatment plans. The role involves prescribing medications, ordering and interpreting diagnostic tests, and providing patient education on disease prevention, health maintenance, and wellness.
The Family Nurse Practitioner will also manage urgent and same-day visits, coordinate care with other healthcare professionals, and maintain accurate, timely documentation in accordance with clinic policies and regulatory standards. Collaboration with the care team, participation in quality improvement initiatives, and commitment to culturally sensitive, evidence-based practice are essential components of this position.
- Candidates should possess strong Nurse Practitioner and Nursing skills, with demonstrated ability to provide family-focused, patient-centered care.
- Candidates should possess solid Primary Care skills, including health assessment, chronic disease management, and preventive care across the lifespan.
- Candidates should possess foundational Medicine knowledge appropriate to advanced practice providers, including diagnostics, pharmacology, and evidence-based guidelines.
- Candidates should possess Urgent Care skills, including triage, management of minor injuries and acute illnesses, and appropriate referral or escalation.
- Current, unrestricted Nurse Practitioner license in the state of Tennessee (or eligibility to obtain), with national board certification as a Family Nurse Practitioner.
- Graduation from an accredited Nurse Practitioner program; master's or doctoral degree in nursing or related field preferred.
- Experience in outpatient primary care or family practice setting preferred, especially in rural or underserved communities.
- Strong communication and interpersonal skills, with the ability to build trusting relationships with diverse patient populations and work effectively in a multidisciplinary team.
- Proficiency in electronic health records, clinical documentation, and basic computer applications.
- Commitment to continuous professional development, quality improvement, and adherence to ethical and regulatory standards.
