Job Title: RN Visit Nurse – Home Health, 1:1 Care + $4K Bonus

Schedule: Full-Time with On-Call and Weekend Rotation
Compensation: $75,000 – $100,000 per year plus $4,000 Sign-On Bonus

- Provide skilled nursing visits in homes and care communities.
- Perform assessments, wound care, and medication management
. - Educate patients and families on care plans and recovery.
- Coordinate with physicians, therapists, and care teams.
- Maintain accurate, timely documentation

- Active Minnesota Registered Nurse (RN) license.
- Experience in home health, med-surg, clinic, rehab, or hospital nursing
. - Strong clinical judgment and independence.
- Comfortable working in a field-based role
. - Reliable transportation.
