Location: Burnsville and Coon Rapids + 35 mile radius from your home address
Schedule: Full-Time | Monday through Friday, 8:00 AM to 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 to $130,000 annually plus $6,000 in first-year bonuses

As a Case Manager, you re the clinician responsible for guiding each patient s care from start to finish. You’ll coordinate services, communicate with providers, educate families, monitor progress, and adjust care plans as patient needs evolve. Your leadership helps patients remain independent, improve outcomes, and provide peace of mind to families.
What You ll Be Doing- Manage an assigned home health patient caseload
- Complete OASIS assessments and individualized care plans
- Perform skilled nursing visits, assessments, and patient education
- Coordinate care with physicians, therapists, social workers, and interdisciplinary team members
- Monitor patient progress and identify changes in condition
- Educate patients and caregivers on medications, disease management, and recovery
- Complete timely documentation using Homecare Homebase (HCHB)
- Ensure patients receive coordinated, high-quality care throughout their episode
- Active Minnesota Registered Nurse (RN) license
- Three or more years of RN experience
- Home health experience preferred
- Strong clinical assessment and critical thinking skills
- Comfortable working independently while managing a caseload
- Excellent communication and organizational abilities
- Reliable transportation for local travel
