Job Details

The Pharmacist Lead is responsible for overseeing the daily operations of a medication therapy management (MTM) pharmacy team of pharmacists and pharmacy technicians while providing high-quality MTM services. This role combines leadership and management of an MTM pharmacy team to achieve clinical service team goals while also executing clinical responsibilities, including comprehensive medication reviews (CMRs), prescriber recommendations to improve patient’s drug therapy, and addressing adherence barriers with patients who receive medications at the Exact Care Pharmacy.

The Pharmacist Lead ensures compliance with all regulatory, vendor contract and internal requirements, promotes patient safety, and drives excellence in service and clinical outcomes.

Responsibilities Leadership & Team Management
  • Develop MTM team strategy and processes to achieve clinical service team goals
  • Manage and supervise daily activities of pharmacists and pharmacy technicians
  • Schedule, train, mentor, and evaluate pharmacy team members
  • Foster a collaborative, professional, and patient-centered work environment
  • Address performance issues, workflow challenges, and staffing needs
  • Lead team meetings and support continuous improvement initiatives
  • Implement and monitor standard operating procedures (SOPs)
  • Handle escalated patient, provider, or operational issues
Clinical & Pharmacist Responsibilities
  • Assess medication appropriateness, effectiveness, safety, and adherence
  • Perform comprehensive medication reviews (CMRs), targeted medication reviews, prescriber outreach and recommendations, and address adherence barriers
  • Provide patient counseling and education on medications and disease states
  • Collaborate with prescribers and healthcare teams to optimize patient outcomes
  • Document clinical interventions and patient care activities accurately
Quality, Compliance & Performance
  • Monitor key performance indicators (KPIs) related to quality, productivity and revenue
  • Ensure adherence to legal, privacy, compliance, contractual, and quality assurance standards and requirements
  • Perform quality assurance review of recorded calls and include in performance management of pharmacy team members
  • Promote operational efficiency and best practices to achieve desired outcomes

Experience
  • Previous experience managing, supervising, leading or overseeing the work of others
  • Experience in a patient care environment
  • Prior interface with at least one of the following preferred:
    Outcomes, Med Monitor, MTMPath, and EQUIPP
  • Prior Managed Care or MTM experience

  • Strong leadership, communication, and interpersonal skills
  • Excellent clinical judgment and attention to detail
