The Gold Care Program is a highly collaborative, multidisciplinary team of physicians, nurse practitioners, nurse case managers, social work case managers, community health workers, and scheduling team, united by our mission to build a better care delivery system for Devoted’s most complex members. We provide whole-person clinical, social and logistical support for patients with complex chronic conditions, frail elders, and patients at high risk of hospital admissions.
This position is an amazing opportunity for an ambitious Advanced Practice Provider (APP) to take on a new leadership role in the Gold Care Program in developing colleagues and improving the effectiveness of our clinical teams. As Gold Care APP Manager, you will be responsible for clinical and operational oversight of a team of Gold Care APPs within Devoted Medical. The position will join the team of Managers and Supervisors across Gold Care (Physician, RN Case Manager, Social Work Case Manager, Community Health Worker, and Outreach Coordinator) and work together to strengthen the program’s collaborative culture, empower the front line staff to achieve clinical excellence, and enhance interdisciplinary relationships in teams to continuously improve patient outcomes.
Leading and learning by example, twenty percent of your time will be designated for patient care.
Sixty percent of your time will be dedicated to the following management duties:
Oversight and accountability for the clinical performance of the APP team, quality reviews, coaching, teaching, and supporting new hires. When a team member does not meet the requirements for the Gold Care APP role, performance management will also be required.
Accountability for key performance indicators for your team, including utilization outcomes (e.g. Observed/Expected events and costs), accurate documentation, and Stars.
Leading and maintaining a strong team culture, being a standard-bearer for collaboration and agility
Oversight of APP schedules including management of time off, monitoring schedule density and Outreach Coordinator team in patient scheduling changes
Shadowing telehealth visits for onboarding Gold Care APPs, including travel to team’s geographies to shadow home visits, as needed
Recruitment, interviewing, and hiring of new Gold Care APP clinicians
Traveling to team’s geographies to shadow home visits, as needed
Twenty percent of your time will be dedicated to interdisciplinary leadership duties:
Working closely with interdisciplinary team co-leaders to identify needs, problem solve, and solution for improvements within the team
Working cohesively as a lead team to meet clinical and non-clinical outcomes
Assisting the teams in working with escalated patient needs
Coordinating and facilitating ongoing education to existing staff and an on-boarding experience for new teammates
Meeting with the department leadership regularly to discuss team performance, successes, and areas for improvement
Twenty percent of your time is devoted to clinical duties. These may include a combination of:
Seeing patients within your Communities including for urgent or complex needs, transitions of care, APP unexpected PTO or departure from Devoted, and as a back up for windowed scheduling
When needed for One Devoted, providing back up support for other Devoted Medical Clinical teams, including Care OnDemand, Transitions of Care, D2
Me, and Primary CareWorking closely with other members of the patient’s care team including their PCP, specialists, and other Devoted team members including pharmacy, clinical nursing, and social care manager, as well as interfacing with family members and caregivers in order to coordinate care for the member and deliver a collaborative care plan.
An active and clear RN and APRN license or Physician Assistant license and/or the ability to get licensed in multiple states is required at time of hire and must be maintained while employed at Devoted Medical
