Dietitian

To provide clinical nutrition support, care and information to patients and staff of Bay Area Hospital. Performs nutrition assessments & reassessments on assigned units. Evaluates/recommends aggressive nutrition intervention, and provides follow-up for continuity of care. Counsels patients and family members in nutrition principles, modifications and food selection adapting plans for individual lifestyle and using appropriate written instruction material. Communicates appropriate nutritional care data through patient medical record.

Documents using the Nutrition Care process and standardized language.
