We are looking for a compassionate Licensed Practical Nurse (LPN) to provide high-quality care to our residents at Avamere Rehab of Coos Bay

- Complete required record keeping in regard to admitting, transferring, and discharging residents.
- Coordinate with nursing staff and support personnel daily to plan shift services, programs, and activities to ensure residents’ care plans are maintained.
- Prepare and administer medications and review medication cards for completeness and accuracy.
- Plan, organize, develop, implement, and interpret programs, goals, objectives, policies, and procedures necessary for providing quality care.
- Provide leadership to nursing personnel and conduct daily rounds of your unit to ensure direct care aligns with residents’ care plans and wishes.
- Participate in facility surveys by authorized government agencies.
- Must possess a nursing degree from an accredited college or university.
- Must hold an active, unencumbered LPN license in this state.
- Knowledgeable of nursing and medical practices, procedures, laws, regulations, and guidelines applicable to nursing care facilities.
- Must chart nursing notes in an informative and descriptive manner reflecting the care provided and resident responses.
- Experience with Electronic Medical Records and computer documentation systems.
- Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
- Must speak, read, and write English fluently.
- Must have an active CPR/BLS certification.
