Bilingual Public Health Home Visiting Nurse
The nurse home visitor provides personal nurse visits to:
- pregnant people
- children from birth up to age 5 years
- children & youth ages birth up to age 21 years who have special health needs
- caregivers of children enrolled in the Babies First! or CaCoon nurse home visiting programs
In this role, the nurse:
- completes comprehensive nursing assessments and periodic reassessments
- provides prenatal, postpartum, pediatric and parenting education and support to families
- conducts caregiver and child growth and development screens and assessments
- serves as a case manager to develop, implement and monitor care plans
- refers/links clients to needed resources
- documents encounters in the client chart, EMR and data collection systems that include: nursing assessments, diagnosis, planning, implementation and evaluation of care.
