Life Care Centers, a leader in senior living and rehabilitation, is seeking a Certified Nursing Assistant to join our Healthcare & Medical Services team. In this role, you will provide compassionate, hands-on care to residents, assisting with daily living activities, mobility, and personal needs while monitoring and reporting changes in condition. You'll collaborate closely with nurses and interdisciplinary team members to support care plans and ensure safety, dignity, and comfort.

Responsibilities

  • Provide hands-on assistance with activities of daily living, including bathing, dressing, toileting, grooming, and oral care for senior residents.
  • Support safe transfers and mobility using gait belts, walkers, wheelchairs, and mechanical lifts as directed by nursing staff.
  • Measure and record vital signs, weight, and other observations accurately, reporting significant changes promptly to the nurse.
  • Assist with resident meals, including set-up, feeding as needed, monitoring intake, and encouraging hydration and nutrition.
  • Maintain resident comfort, safety, and dignity by responding promptly to call lights, repositioning residents, and supporting continence care.
  • Follow infection prevention and control procedures, including hand hygiene, use of PPE, and cleaning of equipment and resident areas.
  • Document care provided, observations, and resident responses in accordance with facility and regulatory standards.
  • Communicate effectively with nurses, therapists, residents, and families to support individualized care plans and rehabilitation goals.
  • Support residents and families with empathy and respect, contributing to a calm, family-focused, and patient-first environment.
  • Participate in shift reports, in-service education, and ongoing training to maintain and enhance CNA skills and knowledge.
