Supportive Case Manager

Columbia Care Services is seeking a Supportive Housing Case Manager to join our Community & Social Services team. In this role, you will provide recovery-focused case management and housing stabilization support for adults living with mental health challenges. Responsibilities include developing person-centered service plans, coordinating access to treatment and community resources, conducting home and community visits, and supporting clients in maintaining safe, stable housing.

You will collaborate with an interdisciplinary team in a supportive, inclusive culture that emphasizes empathy, respect, and trauma-informed care to improve client outcomes and independence.

Responsibilities

• Provide person-centered case management to adults in supportive housing
• Develop, implement, and monitor individualized service and housing stabilization plans
• Coordinate access to mental health, medical, and social services
• Conduct regular home and community visits to support housing retention
• Document services and progress in electronic health records accurately and timely
• Collaborate with clinical and community partners to support recovery goals
• Advocate for client needs and link to community resources and benefits
• Support clients with life skills, tenancy education, and crisis de-escalation
• Participate in team meetings, supervision, and ongoing training
• Uphold trauma-informed, recovery-focused, and culturally responsive practices
