Implement the YMCA's Diabetes Prevention Program, and Blood Pressure Self Management programs across the association and in the community. Support and manage the implementation of the YMCA's DPP, and other Chronic Disease Prevention programs.

• Establish relationships with healthcare providers, public health, physician, and payer communities and other referral networks to enhance awareness of and referrals to YMCA's DPP and BPSM. Oversee Community Health web page and social media pages
• Serve as a liaison, ambassador, and advocate for all YMCA's chronic disease prevention programs within healthcare system, public health, physician and payer communities.
• Respond to inquiries from the general public and members of the healthcare provider, public health, physician and payer communities about the YMCA's chronic disease prevention programs.
• Engage and Support Marketing and Communications department to promote YMCA's chronic disease prevention programs to members of the Y, community members and the Healthcare arena.
• Serve as primary contact and participate in market launch activities.

• Organize all YMCA's DPP and other chronic disease prevention programs. Track program data and outcomes in Redcap and DPP Express
• Recruit, screen and register eligible participants into YMCA's DPP and other chronic disease prevention programs .
• Organize master schedule of YMCA's DPP and other chronic disease prevention programs classes offered throughout the association.
• Engage marketing staff to ensure adequate publicity and marketing of all programs.
• Assist Lifestyle Coaches, staff with launching of each group and evaluation of the group.
• Assist with retention and commitment of program participants.
• Regularly analyze data from YMCA's DPP and other chronic disease prevention programs to ensure it meets quality performance standards.
• Audits YMCA's DPP and other chronic disease prevention programs to ensure compliance with program standards.

Bilingual in English and Spanish

Must have outstanding communication skills; exceptional organizational and administrative skills; a passion for data and details; flexibility, and the ability to build strong relationships, inspire confidence, and work effectively with a wide variety of stakeholders and partners (physicians, healthcare staff, public health officials, employers, payers, health plan administrators, health seekers, and senior Y leaders, lifestyle coaches and branch staff) delivering programs.

Computer

Skills:

Microsoft Office and Personify, Red Cap or other data management programs and others as required. Four-year health sciences related degree preferred but will accept on the job experience in similar programs.
