Job OverviewWe are seeking a dedicated Home Health Field Licensed Vocational Nurse (LVN) to provide skilled nursing care to patients in their homes. The ideal candidate is dependable, clinically confident, and enjoys working independently while still being supported by a strong clinical team.
Responsibilities- Provide skilled nursing care under the supervision of an RN Case Manager
- Perform wound care, medication administration, vitals, and disease process teaching
- Assist with care plan implementation and updates
- Conduct patient assessments and report changes in condition promptly
