You will administer medications and treatments, monitor residents for changes in condition, support individualized service plans, and communicate with residents, families, providers, pharmacies, and community team members.

What You'll Do
  • Administer medications and treatments according to provider orders, nursing direction, applicable standards, and community procedures
  • Observe residents for changes in physical condition, cognition, behavior, mood, appetite, mobility, and daily functioning
  • Obtain and document vital signs and other assigned resident health information
  • Provide supportive and restorative nursing care within the New York LPN scope of practice
  • Collect, record, and report resident health information to the supervising nurse or authorized provider
  • Promptly report significant changes in resident condition
  • Carry out nursing interventions, treatments, and instructions established by the supervising nurse or provider
  • Participate in resident assessments and service-plan reviews under appropriate clinical direction
  • Assist with implementing and maintaining individualized resident service plans
  • Receive, review, and carry out provider orders within the LPN's scope of practice and community procedures
  • Coordinate medication refills and communicate with pharmacies and providers
  • Maintain accurate electronic and written documentation of medications, treatments, observations, incidents, and nursing services
  • Perform medication-cart, treatment-record, and documentation audits as assigned
  • Support resident admissions, hospital returns, transitions, and changes in condition
  • Provide health teaching and wellness counseling within the LPN's training, competency, and scope of practice
  • Promote hydration, nutrition, infection prevention, medication safety, fall prevention, and overall wellness
  • Respond promptly to resident emergencies and urgent needs
  • Perform CPR or Basic Life Support when trained, certified, and required
  • Communicate with families and providers regarding resident needs and changes as directed
  • Participate in resident-care meetings and Wellness team communication
  • Provide day-to-day direction and support to resident-care team members as assigned
  • Reinforce resident service plans, care instructions, and community procedures
  • Monitor assigned resident-care activities and promptly report concerns to the supervising nurse
  • Assist with staff training, competency reviews, audits, and required in-service education
  • Maintain resident privacy, dignity, rights, and confidentiality
  • Complete additional nursing and Wellness duties within the LPN's lawful scope of practice
What We're Looking For
  • Current and valid New York State LPN license
  • License must be active and in good standing
  • Knowledge of practical nursing standards, medication administration, infection prevention, and resident safety
  • Strong observation and resident-monitoring skills
  • Ability to recognize, document, and report changes in resident condition
  • Ability to work under appropriate clinical direction and seek guidance when nursing assessment or independent professional judgment is required
  • Strong documentation, organization, and time-management skills
  • Comfortable using electronic health records or resident-care documentation systems
  • Ability to communicate professionally and compassionately with residents, families, providers, pharmacies, and coworkers
