Hepatitis C Case Manager

Job Description

As a federally qualified, non‑profit health center serving communities in Suffolk, Rockland, Orange, Dutchess, Ulster, Sullivan, Columbia and Westchester County, we are currently seeking a full‑time Care Manager.

The applicant will work with the Director of Nutrition and Operations to provide case management for patients diagnosed with Hepatitis C (HCV).

• Facilitate enrollment in HCV benefits and entitlement programs
• Conduct adherence assessments and evaluations with patients diagnosed with HCV to reduce barriers to care
• Provide case management, treatment adherence, and support services for patients diagnosed with HCV
• Provide appropriate follow up on referred patients
• Participate in case conferences as well as other agency or community‑based meetings or trainings
• Maintain data and case records as required and prepare necessary reports
• Work cooperatively and collaboratively with the entire HCV case‑management, medical and behavioral health provider teams

High School Diploma/GED required.

Full‑time
Pay: $23.00 - $24.68 per hour
