- n this role, you will provide evidence-based physical therapy to medically complex patients in a Long-Term Acute Care Hospital (LTAC), helping them regain strength, mobility, and independence following serious illness, injury, or surgery
- The Physical Therapist evaluates, plans, and implements individualized treatment programs for adult and geriatric patients while collaborating with physicians, nurses, case managers, respiratory therapists, occupational therapists, and speech therapists to maximize each patient's recovery and quality of life
- Evaluate adult and geriatric patients and develop individualized treatment plans based on physician orders
- Provide physical therapy services designed to restore function, improve mobility, reduce pain, and prevent disability
- Perform therapeutic interventions including therapeutic exercise, gait training, transfer training, balance activities, neuromuscular re-education, manual therapy, and functional mobility training
- Conduct diagnostic and prognostic assessments of muscle strength, joint mobility, balance, coordination, and functional abilities
- Supervise and direct Physical Therapist Assistants (PTAs) in accordance with state practice guidelines
- Educate patients and families on home exercise programs, safe mobility techniques, posture, transfers, and the use of assistive devices
- Recommend and train patients in the use of walkers, canes, crutches, wheelchairs, braces, and other adaptive equipment
- Modify treatment plans based on patient progress and changing clinical needs
- Maintain accurate, timely, and complete documentation in the electronic medical record
- Participate in interdisciplinary care conferences and discharge planning
- Maintain compliance with hospital policies, infection prevention standards, patient safety initiatives, and regulatory requirements
- Participate in departmental meetings, continuing education, and performance improvement activities
