You will coordinate and oversee care for high-acuity patients, collaborating with physicians and an interdisciplinary team to implement individualized care plans.
The ideal candidate holds an active RN license in the state or a compact state, with 1+ year of case management experience preferred. BLS is required; ACLS within 90 days of hire.
