Location

Responsibilities
  • Utilize the nursing process in assessing, planning, implementing, and evaluating patient care.
  • Provide culturally sensitive care appropriate for the patient’s developmental age.
  • Interact effectively with patients, family members, and other health team members; follow the nursing plan of care and maintain professional nursing standards.
  • Contribute to the development of the nursing plan of care through planning, data collection, implementation, and evaluation of the patient’s care.
  • Perform basic nursing skills and treatments as prescribed by the physician or as indicated by the patient’s condition.
  • Provide quality patient care services aligned with the standards of care established by Nursing Services and CRHS.
  • Function as part of an interdisciplinary team with positive interpersonal communication with patients, families, visitors, physicians, and other staff.
  • Document patient care with appropriate nursing interventions and evidence of patient/family education.
  • Implement and document patient responses to treatments within the scope of LPN duties and CRHS policies.
  • Demonstrate prioritization and decisiveness to complete typical patient assignments within a shift.
  • Perform duties related to safety, infection control, and performance improvement as part of CRHS’s quality care.
  • Conduct rounds every two hours to provide quality care and services.
  • Presents a professional image to colleagues, staff, patients, families, and visitors.
  • Attend professional development programs and in-service training; meet continuing professional education requirements.
  • Perform other related duties as assigned to assure resident safety.
Standards of Performance
  • Perform nursing care in accordance with department and CRHS policies and procedures, as observed by nursing, medical, and management staff feedback.
  • Complete assigned patient care duties in a timely and accurate manner as measured by logs and charts.
  • Adhere to safety and infection control policies in patient care.
  • Demonstrate reliability as measured by adherence to CRHS Time and Attendance policy.
  • Maintain professional conduct and compliance with safety, confidentiality, and HIPAA standards.
  • Promote teamwork within the department and with external contacts as observed by managers and coworkers.
  • Adhere to CRHS Standards of Behavior as observed by managers, coworkers, and customers.
  • Verbalize and demonstrate core competencies of the unit and department.
  • Comply with regulatory requirements, including National Patient Safety Goals, as observed by peers and supervisors.
  • Actively participate on the healthcare team by attending mandated in-services and departmental meetings as determined by the Director.
Qualifications
  • Basic Qualifications:

    Requires up to one year of work-related experience or an equivalent combination of education, training, and experience.
  • Licensure, Registrations & Certifications: Requires a current license as a Licensed Practical Nurse and a current CPR certification.
Licenses & Certifications
  • Current LPN license
  • Current CPR certification
